Coaching Client Affidavit

I acknowledge and agree to the following terms for coaching services with Career Maven Consulting:

1. I understand that coaching services with Career Maven Consulting is a professional engagement focused on personal and professional development. This relationship requires my active participation, honest communication, and commitment to growth.

2. I confirm that I will:

   • Arrive on time for scheduled sessions with Career Maven Consulting coaches

   • Come prepared with discussion topics and goals

   • Maintain open and respectful communication

   • Take responsibility for my personal progress and decisions

   • Provide timely notice for session cancellations (at least 48 hours in advance)

3. I recognize that coaching services provided by Career Maven Consulting is not therapy, counseling, or medical treatment. I affirm that I am mentally and emotionally capable of engaging in coaching services at the time of my appointment and will disclose any conditions that might impact my full participation.

4. I agree to maintain professional boundaries, treating my Career Maven Consulting coach and our interactions with mutual respect. I understand that inappropriate, offensive, or harassing language or behavior will result in immediate termination of coaching services.

5. I consent to the confidential nature of our coaching relationship, understanding that personal information shared will be kept confidential within professional and legal limits.

By checking this box, I voluntarily agree to these terms and commit to a professional, constructive coaching experience with Career Maven Consulting.
